A) ART Scene Shop - (Team 9, Lenny Solomon)
- In general, the scenery shop was in very good shape, the following issues were noted:
- All of the circuit breakers are not labeled, although the main power shut-offs were tagged.
- There is still just one fire alarm pull for the entire area. It was suggested that signs be installed that indicate the location of this pull.
- The cold saw should have a power shut-off box (breaker box) installed for the times the saw requires maintenance.
- Oil should be stored in a flammable cabinet.
B) Hoffman basement - 2nd floor - (Team 12) read by Lenny Solomon
- Rm. 228: Users take shoes off when working in lab, working with chemicals. Should discuss this practice further. Maybe furnish users with laboratory only shoes, or cleanroom shoe covers? Install plugs in the recirculating chilled water lines (in case valves open or start leaking).
- Rm. 204: No fresh air supply to lab. Add plugs to recirculating chilled water. No usable telephone in lab. Moniter on floor blocking access to electrical panel. No sign indicating location of fire extinguisher. Maybe should install sign indicating high magnetic field (6K gauss)?
- Corridor: Gas cylinders not chained/secure.
- Rm. 236: No eye protection. No fire extinguisher sign. Flammable gas cylinder not chained/secure. Need sharps containers.
- Rm. 211: Need sharps containers. Acids (sulfuric, nitric, HF) not stored in cabinets. Hood needs to be inspected.
- Rm. 233: Laser in use sign not obvious. Need fire extinguisher signs. Need sharps boxes. Need eye protection.
- Basement (B017, B004): Need signs indicating location of fire extinguishers.
- Rms 110A-120 Langmuir Lab. Very good. Just need secondary containment for chemicals and eye protection.
- Rm. G28A: Eye wash station last tested in 2003.
C) Jefferson Labs - (Team 5, Alex Donohue)
- General Comments
- Most of the labs (150, 158, 160, 161) lacked clearly visible fire escape plans.
- Many secondary containers had too small of a volume to contain the spill of even one bottle.
- Room 150:
- There were a large number of solutions in one of the back rooms. Although these were reported as being in active use, the large number and stacking indicates this may have been permanently stored here.
- Broken sharps container was full.
- Secondary waste containers were too small for the bottles in the SAA.
- Room 160:
- One of the fire extinguishers was inaccessible due to objects placed in front of it.
- Several solvents were stored permanently on the countertop.
- About 70% of the fume hood was occupied by chemical bottles.
- Room 164: (Golovchenko)
- Cyanide solution was found sitting on an open wooden shelf with no secondary containment! Need cyanide antidote.
- Water bottles and empty soda cans indicate that beverages for human consumption are used and left in the lab.
- Batteries were placed close to chemicals.
- Room 167:
- Solvents were not stored in secondary container.
- Developer under the hood was not in a secondary container
- Acids were sitting on top of a secondary container rather than in it.
- Larger secondary containers would be useful in the SAA.
D) HEPL & 60 Oxford Street - (Team 11, Parisa Fallahi) the inspection has not been done yet. The report will be presented at the May meeting.
A) Lyman Labs:
- Silvera Group
- Room 130:
- HF antidote out of date - a new one has been purchased.
- hood extremely cluttered, - this has been cleaned up.
- poorly labled cynide on shelf (not in MSDS) - relabeled.
- alcohol bottle (4L) on shelf, no secondary container - this has a new container.
- Room 128:
- fridge hanging from crane by old, very tight rope - should use metal cord. - this is very stable.
- Unsecured gas cylinder in “clean room” - secured
- Lukin Group
- Room 230:
- laser light not working - this is fixed.
- coffee maker in lab - ok, it is on a sink used for food.
B) ESL - all ok
- 111 - 304 - all is ok
- 303: Fume hood last inspected in 11/03 [It was recently inspected - Ed Jackson] - the inspection failed and the hood will be retested.
- 305: ok
- 317: ok
- 323: Fume hood last inspected in 11/03 [It was recently inspected - Ed Jackson]
- 324: (Scott Martin Group) Unlabeled, unknown liquid in cabinet (clear, uncapped) - must be labeled.
- 325 - 415 - all ok
C) McKay Labs
- 113 McKay (CIMS Facility)
- Fume Hood: Unlabeled beakers of chemicals. - will be cleaned.
- Waste in need of disposal. - corrected.
- Gas cylinder needs cap. - corrected.
- 109 McKay (Golovchenko)
- Unlabeled beaker in Fume hood- corrected.
- 106E McKay (CIMS Facility)
- Mixed inorganic and inorganic solvents under fume hood- will be cleaned.
- Basement of McKay
- Four 50 gallon trash barrels filled with dirty water from leak - Ed will check on this.
- Boxes on floor soaked with water - Ed will check on this.
- No eyewash tag - Ed will check on this.
D) Third Floor McKay
- 304: (Venky's Group)
- Hallway closet adjacent to room has haz-mat not stored correctly - this has been cleaned.
- 303:
- Hood inspection out of date - called for inspection, Ed Jackson will look into it.
- Eyewash station check form - needs an inspection tag.
- Hallway closet adjacent, cylinders not tied down - this has been fixed.
- 312A - 318 - all issues have been dealt with (received detailed email from Kyle Alvine addressing all problems).
E) McKay 418 - 424 - all is ok.
A. Doyle Group (Lyman Lab) - the safety rep is in Ghana and was not present to give follow-up report!
- Room 31:
- no food sticker on fridge
- power strip in fume hood, where is spill kit
- Room 29:
- laser light is on, laser is not on
- loose gas cylinder
- Room 27: laser light is on, laser is not on
- Room 24 A, B, C:
- no sign on door
- storage on top of flammables cabinet.
- small propane torches on high shelf not secure, edge of shelf.
- C/A:
- unmarked pit needs caution strip
- gas cylinder on table next to computer not secure
- tape cords down, trip hazard
B. McKay Labs
- 221 McKay (Tinkham) - Steve Shepard was not at meeting to give report.
- Broken Bell Jar and Broken Glass on Bench
- 225, 225A McKay (Tinkham) - Steve Shepard was not at meeting to give report.
- In fume hood are full waste containers, they need to be dated and removed.
- Hood needs to be recertified.
- General clutter in room, cylinder tanks in pathway.
- Sink not accessible and filled with dirty glassware.
- Cylinder needs cap
- Beaker with dried material on bench without label appeared to have been there for a long while.
- Cords at head level, suspended from lights.
C) McKay Cleanrooms - Steve Shepard was not present to give follow-up.
- 222 - clean room
- eye wash log not up to date
- no safety goggles
- TCE waste in plastic bottle without a hazardous waste tag
- flammable solvents next to hot plate in hood
- 232 - service chase
- fire extinguisher inaccessible from one end of room, blocked by liquid Nitrogen tank
- unsecured gas cylinders – corrected
- unsecured empty short gas bottle - protective cap was off and there was no visible strapping mechanism available for short bottles
- 3B – clean room
- unspecified laser is being used for writing - the class of the laser should be noted and permitted - interlocks or more effective shielding should be put in place to prevent injury
- no laser safety glasses
- new hood that has not been inspected
- no inspection sticker on hood
- eye wash log last entry 1/29/04 – no drain
- insufficient labeling of chemical storage areas
5. EH&S Report
Rob reported on several people needing retraining.
