Inspection reports were presented by Teams 3, 6, and 7 as follows:
a) Team # 3 - Cruft and Pierce Halls (Ce Wang)
Lab inspection of Cruft/Pierce took place on 1/11/2001
- Cruft was still in the middle of renovation when we did the inspection, and a lot of new renovation was going on. Equipments were moved and many rooms were used as storage space.
- Pierce Hall - We inspected the lab area on 1st floor, the basement and subbasement. Overall, Pierce was in good shape in general. So we could only identify some tiny problems:
- Rm B-7 and B-8: A wooden platform on the floor is above the normal level, caution tapes are needed around it.
- Rm G18: Counter is messy. Cassette tape player is put among the bottles on the same counter. A little compressed gas cylinder is not fastened.
- Many basement rooms in Pierce are in construction.
b) Team # 6 - ESL/HEPL (Andrea Santoro)
ESL:
- Loading dock - Need lower brackets for small gas cylinders (There are plans to install these soon.)
- Basement- water on the floor, most likely leakage from outside
- Weitz group:
- Room 205C - Perhaps put door signs on both doors. Where is the laser that the laser sign refers to?
- Room 213 - Need a sharps box for pipets, etc. Need a laser warning sign on the door. Cords running across floor are a tripping hazard.
- Room 214 - Pipe running across floor is tripping hazard, perhaps could be covered with a ramp
- Room 215 - Cap off two unused water pipes. Refrigerator is not labeled
- Room 215a - another unused water pipe should probably be capped
- Room 216 - no violations
- Room 301 - no door sign, so are not sure who the PI is. Fire extinguisher out of date. Fume hood out of date.
- Mitchell group:
- Room 203 - locked, could not access
- Room 302 - check gas outlets, faint gas smell present. Bases and solvents in same secondary containment tray
- Room 303 - fire extinguisher out of date
- Room 304 - partially full chemical bottles sitting in sink. Bottles of chemicals sitting on counter with no secondary containment
- Room 305 - needs satellite accumulation area. Fire extinguisher out of date
- Room 317 - no violations
- Room 328 - some sort of cylinders of organic gel sitting in sink.
- Martin group:
- General note - new labs, all need door signs
- Room 323, 324, 325 - no violations (except door signs as noted above)
- Room 326 - acids stored on counter with no secondary containment. Hazardous waste tags not completely filled out
- Safety showers outside rooms 205c, 301, 323
- Eyewash stations in rooms 205c, 215, 216a, 324, 325, 326
- Spill kits in rooms 205c, 302
HEPL:
- Atlas - Room 102 needs door sign
- Yezozlinsky - Room 106 needs door sign. Room 306 needs door sign
- Basement - access to electrical panel blocked. Need new flammables cabinet (a new one is on order). Refrigerator needs label (in Doyle area). Cords on floor are a tripping hazard (in Doyle area).
- Safety shower in basement
- Two spill kits in basement
c) Team # 7 - Link Building, Anderson/Wofsy Labs (Bivash Dasgupta)
Anderson Labs:
- Rm 129: Poisonous gas cylinders were kept under the hood. A sign indicating the cylinders are stored there might be a helpful warning.
- Rm 132: (electronics lab) No sign for compressed gas cylinders.
- Rm 125: One of the fume hoods (just outside the machine shop) has no certification. (Hood B) A bottle of organic solvents (I think it was Isopropyl alcohol) was stored along with acids under the hood beside the SAA area. Should be kept in the Flammables storage.
Wofsy Lab:
- Boxes/computer are kept in front of a refrigerator and the flammables cupboard. There is stuff in the refrigerator which hasn't been used for a long time. If it is not essential or not used it should be disposed of.
a) McKay Labs:
- 1st Floor - all major problems have been taken care of
- 2nd Floor - noone was present to report on this area
- 3rd Floor - most items were taken care of. Warning tape has been placed over the cable on the floor.
- 4th Floor - all concerns have been fixed. Room 427 does not need an eyewash. This issue is raised every year during inspection, always with the same result.
- 5th Floor - all issues corrected. Perhaps there should be a warning sign posted near the UV light.
- All lasers on 4th and 5th floors have interlocking lights
b) Lyman Labs:
- Basement - The hallway area is a mess due to construction. The spill kit was taken care of. Also did a general clean-up of the area.
- First Floor - All issues have been taken care of. There are plenty of safety goggles available for use in the lab. The old chemicals are ok.
Rob read a list of names of people who need to update their online training in March.
Andrea del Vecchio asked if we should provide training on the hazards of perchloric acid which is highly explosive. Lenny said he would send email to the department to see who is using it. He will then train them ad hoc.
