C. McKay Cleanroom (Marc Durand)
- Basement Cleanroom:
- Labels with list of chemicals are missing at the entrance door
- Need a tag on the eyewash station indicating the last time the tap had been opened for at least 3 minutes
- 2 unknown glasses with chemicals on a table
- Corrosive liquids should be stored in a separate cabinet
- There is no fire extinguisher in the cleanroom
- There is a laser, protected by tinted glass, but there is no sign or light to indicate if the laser is in use or not. Is there an automatic shut down of the laser when the window is opened?
- In the corridor, there is a bottle of oxygen without a tie or chain.
- 2nd Floor Cleanroom:
- Labels with list of chemicals are missing at the entrance door
- Need a tag on the eyewash station indicating the last time the tap had been opened for at least 3 minutes
- Need to put labels on all the cabinets indicating what kind of chemicals are in there, or that it is an accumulation area.
D. Cruft/Pierce (Matt Eisaman)

- Acid and solvents should be kept separately. ... It was done while we were inspecting.
- Room 116 - It was unclear if the red fire extinguisher was charged.
- The written fire emergency plan was outdated.
- Cruft 109/110/111 (Hau)
- Room 110/111 - The high voltage switches were labeled but the outlets were not labeled.
- Room 110 - Emergency access plan was out of date (L. Vernac)
- Lyman 22/23 (Hau)
- Doesn't have a spill kit.
- Waste acid and solvent should be stored separately.
- Fire extinguisher was located behind a coat rack.
- Lyman 23 - The fire extinguisher was not inspected in the past year.
- Need goggles for photoresist spinner.
- No broken glass containers in either room.
- Cruft 312 (Capasso)
- The written fire emergency plan was outdated.
- No eye wash station.
- Cruft 219 (Electronics shop)
- Update emergency access sheet (Bill Walker's info).
- Need sharps container.
- Some old glues and chemicals need cleaned out.
- Pierce B2-B7
- The gas cylinders are not arranged properly. Some of the cylinders are kept on shelf. These are to be removed and chained properly. Unused cylinders need to be capped.
- Empty bottles of chemicals need to removed.
- There are unused cans of chemicals sitting in the cabinets, need to be removed.
- No eye wash and safety shower in the lab; definitely need one for a big lab like this one.
- There were no high voltage warning signs for 220V and 2kV power supplies.
- The fire extinguisher was not inspected in the past year.
- Pierce B7, B7b, B7c - There was no written fire emergency plan.
- Pierce B12 - OK
- Pierce G7A
- Separate space for satellite accumulation needed.
- Separate cabinets for acid and solvents needed
- Chemical spill kit not available.
- The circuit breaker was blocked by a toolkit.

- Refrigerator needs to be labeled.
- Lab in the process of being built.
- There is no emergency shutoff or breaker box
- Pierce G11,G12A (Senior project lab),G13 - OK
- Pierce G16 (machine shop) - Only 1 pair of goggles
Note: John Budney will report on laser/radiation violations (not many) when he returns from Brazil (at March meeting).
3. Lab Inspection Follow-up Reports
A. ESL and HEPL:
- ESL 215 - empty containers (~20) need to be removed - Not a problem; this is ok.
- HEPL 104 - mark chemical fridge as such - This has been done.
- HEPL 105 - need interlock for laser, emergency contacts. - This is being worked on.
- HEPL 106 - power cords hung from ceiling - This issue has been fixed.
B. Maxwell/Dworkin:
- MD 338:
- Small unlabelled squirt bottle on shelf above sink - This is ok.
- Silicone precursors stored in unlabelled cabinet - need to list hazards - This has been taken care of.
- Silicone precursors not listed on door sign - This has been taken care of.
- Door sign missing on the second door to the lab - This has been taken care of.
- Small (Class IIIA) laser no longer in use. Can it be boxed up and taken off the door sign? - The laser should be and will be packed up.
- MD 311 – VLSI lab:
- Need door signs on 311 and 313 - No one was present to follow up on this issue!
- Need to list radiation sources on door signs - No one was present to follow up on this issue!
- MD 321:
- Small fridge needs a “Food Only” sign - No one was present to follow up on this issue!
- MD 133:
- Hood mentioned on last year’s report appears to be empty and unused. - This is not a problem.
C. Link Building (Anderson Group)
- Basement area - lots of clutter; dripping oil - The pumps are being repaired.
- Machine Shop - there are no goggles available in room - The goggles are there. Workers in lab know where they are.
- Fume hoods - need to be inspected - Cathy Thomas will inspect the hoods.
Rob read a list of names that need retraining.
