A. McKay Floors 1-3 (John Budney) - Inspection is not finished. John Budney's team will try to finish floors 2 & 3 by next month.
- Rooms 125 and 124 - These are lab areas that are no longer in use and are waiting for someone to use them. They are currently being used as storage and are disorganized and cluttered. There are still lab related items in these rooms including a flammables cabinet which is not empty.
- Room 122 (PI - Markus)
- Emergency access sheet? Could not find
- Fire extinguishers were not obvious….Could not find.
- Satellite accumulation area??? There was a satellite accumulation area sign on a pump closet door in the outside hall.
- Basement clean room
- Laser in mask maker…No sign of radiation sticker.
- MBE Machine blocking one of two exits.
- Electron Microscopy Lab (CIMS) No PI
- No date on the emergency access sheet…
- Flammables Cabinet was not labeled
- Did not notice a fire emergency plan
- McKay 113
- Roughing pump oil spill on floor. Clean up spills immediately. Consider
putting roughing pumps in secondary containers- Propane canister found in a wooden cabinet with dry chemicals, oils and
sealant. Should be stored in a flammable cabinet.- Unmarked old rusty containers stored in wooden cabinet with dry
chemicals, paints, etc.- Small neon gas canister not strapped down. Also sitting right next to
room's heater.- Front fume hood is very messy and the flow of air may be obstructed
- Back fume hood used for chemical storage.

- This room was discussed after last year's inspection and remains unchanged this year!
- Pierce B12 - Green liquid in a hole in the ground!!
NOTE: Lenny reminded the committee that the "green liquid in Pierce B12 was tested last year. It was determined that the liquid is NOT hazardous waste and can pumped out or drained.
E. American Repertory Theater (Steve Shepard)
- There were NO significant safety issues at this location.
- The workers at this site would like prescription safety glasses. Lenny said he needs to look at documentation that explains exactly what kind of safety apparatus Harvard is obligated to provide to its workers. Mike Labosky will get some info on this for Lenny. Snap-on safety shields can be worn over regular eyeglasses; however, A.R.T. people want/need something more substantial. It was mentioned that the Chemistry Dept. reimburses people up to $150 per person for safety glasses.
- Respirators - A.R.T. wants to streamline the respirator testing process. Can testing and training be done on-site? They do not want to have to make several trips to UHS, etc. Mike Labosky said that EH&S could arrange to test/train people on-site to use the respirators; however, they will still need to visit UHS or their doctor to get their breathing tested. There is no way around that.
COMMENTS: Ed Jackson said he can supply fire extinguishers to any lab area that needs them. Just contact Ed.
3. Lab Inspection Follow-up
Maxwell/Dworkin (Ben Peirce): there were no significant issues needing attention. He did put signs on the refrigerator.
Rob read a list of names that need retraining.
