Safety questionnaires have been distributed throughout the SEAS and Physics for a number of years. The information gleaned through this process is used to help define the training requirements. This committee reviews and revises the questionnaires. The current questionnaire is available. If you are new to the department, please fill out this questionnaire and submit it.
